Please Bring the Following Information to Your Appointment:
- All radiology studies pertaining to your current medical problem. They must be taken within the last 12 months and must include either X-rays, MRI or a CT scan.
- Prior medical records, such as operative reports, EMG and nerve conduction results. Also include consultations from other physicians, particularly those from your referring physician.
- A complete list of all medications you are currently taking.
- Your insurance card, co-payment, and billing form. You may want to contact our billing company, MedNet, at (310)322-4278 if you have any questions regarding coverage by your insurance plan.
- If Worker’s Compensation is involved, then written pre-authorization is required by your employer’s insurance company. Please bring the written pre-authorization with you to streamline the paperwork and payment process.
- The names, phone numbers, and addresses of ALL physicians involved in your care. Please be sure to include primary care physicians.
- The completed New Patient Questionnaire (click link to display for printing.)
- The completed Pain Drawing Form (click link to display for printing.)
- The completed Functional Questionnaire (click link to display for printing.)
- The completed General Review Form (click link to display for printing.)
